Cancellation Policy
Your appointments are of utmost importance to us at Morph Health and Wellness. They are reserved with our providers to meet your specific needs. We understand that unexpected events can occur. In our commitment to efficiency and providing the best possible care, we adhere to the following policies:
Late Arrival and
form completion Policy
If you are 5 minutes or more late to your appointment without prior notice and approval or do not respond to the provider’s call as prompted, your appointment will need to be rescheduled. We require at least 24 hours’ notice to change or cancel appointments. A ‘no show’ or cancellation without 24 hours’ notice will result in a charge for the full fee of $100.00 for your appointment. If the required patient paperwork and consent forms are not returned before your appointment, your visit will be considered a cancellation and result in a charge for the full fee of $100.00 for your appointment.
Missed Appointment and Cancellation/No-Show Fee
If you cannot keep a scheduled appointment with your provider, you must provide a minimum of a 24-hour notice. If you fail to manage your appointment online or contact the office to cancel or reschedule your appointment before 24 hours of your scheduled appointment, you will be charged a $100.00 missed appointment fee to the card on file.
